Inflammatory Bowel Disease (IBD)
Exploring current medical nutrition therapies in IBD
The presentation covers:
- Overview of IBD:
- Definitions, types (Crohn’s disease, ulcerative colitis, unclassified IBD, microscopic colitis), prevalence and diagnosis challenges.
- Causes and risk factors: genetics, environment, immune response, urban diets, microparticles, and the “genes load the gun, environment pulls the trigger” concept.
- Symptoms and complications: abdominal pain, diarrhoea, anaemia, malnutrition, psychological impact.
- Medical treatments: corticosteroids, immunosuppressants, biologics, JAK inhibitors, S1P modulators, and surgical options.
- Advances in diagnostics: bedside ultrasound, enhanced endoscopic techniques, cancer surveillance.
- Nutrition therapies:
- Exclusive Enteral Nutrition (EEN): gold standard in paediatrics, emerging evidence in adults, Logan Hospital trials.
- Crohn’s Disease Exclusion Diet (CDED) and CD-Treat approaches.
- FO-SHORE diet in UC, low FODMAP diet for functional symptoms, Mediterranean and plant-based diets.
- Evolving role of fibre, prebiotics, and probiotics (limited role in Crohn’s, selective in UC).
- The EEN paradox: why a processed formula works despite links between ultra-processed foods and IBD.
- Nutrient deficiencies: iron, B12, vitamin D, calcium, zinc.
- Practical dietetic strategies: supporting adherence, tailoring plans, bridging to surgery, patient support.
- Future directions: AI in diagnostics and flare prediction, microbiome research, nutrigenomics, metabolomics, digital health tools, and psychological support.
- Emphasis on multidisciplinary care: dietitians, gastroenterologists, IBD nurses, and psychologists.
