NDIS Nutrition support: case study
The case:
Jenny is a 50 year old woman with cerebral palsy and an NDIS participant. Her nutritional and hydration needs are met entirely by PEG feeding. She is prone to be overweight and has GORD (reflux) and constipation. Jenny has required multiple hospital admissions in the past for wound healing, urinary tract infections and aspiration pneumonia.
Carmel uses the NDIS Participant Goal: To be Healthy and Well to describe how input from dietetic services can impact on functional and social outcomes for Jenny.
Jenny’s goal To be Healthy and Well is specifically addressed in the webinar using social and functional outcomes in each of the following areas:
Choice and Control
Independent Living
Better Health and Wellbeing
Social and Community Participation
Capacity Building and Skills Development
Carmel then takes us step by step in detail explaining the four parts to her NDIS justification for Jenny:
Dietitian Report (health and clinical outcomes)
NDIS Justification in Social and Functional Outcomes
NDIS quote for ongoing service
