Food Protein-Induced Enterocolitis Syndrome (FPIES): case study

The case:

A six month old baby experienced profuse vomiting two hours after commencing rice cereal for the first time. In the emergency department he was diagnosed with gastroenteritis and discharged. A week later at home he was given rice cereal again which resulted in severe vomiting after an hour. He was readmitted to hospital pale, unresponsive and floppy. FPIES is a rare condition which is often not diagnosed at the first presentation. Vicki explains how FPIES is diagnosed, which are the most common trigger foods and the medical management. Vicki describes the dietitians role in the management of FPIES in detail including:

  • Having FPIES on our radar with paediatric clients who have a history of vomiting
  • an introduction of solids plan for the baby with FPIES
  • the ASCIA FPIES action plan
  • other issues these babies may have which can include; aversions to trying new foods and textures, being spoon fed, sensitive gag reflexes and the families are often understandably anxious
  • recommendations around food challenges
  • follow up care

About the Presenter

Vicki is an Advanced APD and the dietitian for the Department of Allergy and Immunology at the Royal Children’s Hospital in Melbourne, where her role involves patient care, teaching, training, resource development and research. Vicki is involved in food allergy training of dietitians, nurses and doctors and is a frequently invited to participate in conferences, training sessions, and resource development in the area of food allergy. She also has a private practice specialising in food allergy at the Melbourne Allergy Centre and Children's Specialists, based in Parkville. She is the past chair of the dietitian subcommittee of the Australasian Society of Clinical Immunology and Allergy (ASCIA). Vicki is involved in food allergy research and has several publications related to food allergy and is completing her PhD with the Centre for Food and Allergy Research based at the Murdoch Children’s Research Institute and the University of Melbourne.
